To those with medicare, I will be a provider for Medicare when approved by the House of Representatives. Right now medicare only accepts acupuncture treatments from board certified physicians - not acupuncturists.

1. Copies of Cards, both sides, current

2. Address and Date of Birth

3. Please, it’s imperative: Call your insurance company to affirm coverage, number of visits, and co-payments

Patient is responsible for payment. Co-pay may be required and is payable with cash, check or credit card and is due at the time of treatment. Patient is responsible for all costs incurred.
